I hereby affirm that I am 21 years of age or older and that I am registering under my legal name. I understand that I will be required to provide photo ID and that I may not be admitted to a F.L.O.G. event without it. I understand that my registration is NOT transferable to any other person under any circumstances.

I understand that Rules, Etiquette and Protocols following RACK (Risk Aware Consensual Kink) have been set for F.L.O.G. events, and that I am required to follow them while attending a F.L.O.G. event. Recognizing that there is an inherent risk now of exposure to COVID-19 that exists in any public place where people are present, I agree to abide by all F.L.O.G. Rules, Protocols and Etiquette which I have received and read.

I agree that everything that I observe at F.L.O.G. events will be kept in strict confidence, and that I will do nothing to jeopardize the privacy and identity of any other attendees. Further I agree that NO photographs or recordings shall be made of anything at F.L.O.G. without the expressed written permission of F.L.O.G. and any individual attending the events.

I state that I attend F.L.O.G. events for my personal enjoyment and that I understand that in doing so I will be exposed to BDSM activities that some people might find offensive, and that I consent to being exposed to such activities at F.L.O.G. events. I further agree to not interfere with others’ BDSM activities, and to take any concerns I have to the event organizers.

I certify that I am not an employee of any law enforcement agency, or that if I am, I am attending F.L.O.G. events solely for my personal enjoyment and not as part of any investigation. I certify that I am not a member of the media, or that if I am, I am attending F.L.O.G. events solely for my personal enjoyment and not as part of any investigation or gathering of information and that I will not use any such information in any manner without the express written permission of F.L.O.G. and/or any individual attending the events.

I agree to hold harmless and indemnify the owners/operators of any location F.L.O.G. should meet in, F.L.O.G., and any and all of its volunteers, agents and/or assigns from any and all liability, direct, indirect, subrogated or not, vicarious, or otherwise, which may accrue as a result of any accident, injury, cause of action, or damage to my person or property, or any other damage of any kind as a consequence of my attendance at F.L.O.G. events.

I state that I have read and agree to these statements, and that by attending a F.L.O.G. event, I voluntarily assume all risks of doing so and all risks related to possible exposure to COVID-19.
